To Make a Referral
Specific referral steps differ considerably depending upon the age of the individual being referred. Interested persons are encouraged to call our admissions office for more information. When you call, we will obtain information about the child, answer any questions you may have about our programs, and guide you through the referral process. Typically, it is the responsibility of the county or local school district in which each child resides to make a formal referral to Summit. The following is a brief summary of how the process may work for you:

Birth - 2 Years Old - An application for evaluation or services must be approved by the county in which the child's resides.

3 - 4 Year Olds -- The child's local school must approve an application for evaluation or services.

5 - 21 Year Olds - An application for placement or evaluation must be approved by the child's local school district through its committee on special education.

Family Support Services - Many of Summit's family support services are funded through a state program called Medicaid Waiver Home and Community-Based Services. A child or adult with a diagnosed developmental disability (evidenced before age 21) may qualify for these services and a call to Summit can start the process.
