Service Coordinators assist individuals in search of the supports and services that they require or desire, in order to facilitate independence in their community.  Caring for a family member with a disability requires a great deal of help from extended family, friends, and professionals.  With this knowledge, Summit’s highly-skilled professionals also help families and caregivers find their way through the myriad of programs and services available to children and adults with disabilities.  Service Coordinators work with individuals and caregivers to determine eligibility for services, help identify and link individuals and their caregivers to area programs, and monitor and coordinate services and activities.  Service Coordinators provide support and advocacy in many areas of an individual’s life. This service is funded by the New York State Office for People with Developmental Disabilities (OPWDD). Certain eligibility requirements apply.
